Dive shop check-in documents commonly include proof of certification, the standard diver medical questionnaire, a signed liability release and often proof of insurance. Requirements vary by operator. Missing paperwork can delay or cancel a trip.

Prepare the documents your operator requests:

• Proof of certification.
• The standard diver medical questionnaire. A “yes” answer to certain questions requires a physician’s approval.
• A signed liability release.
• Proof of insurance, often requested for liveaboards and overseas trips.
